Please read this form carefully and be aware that by registering for and participating in this program or by registering for participation in this program, you will be waiving your rights to all claims for injuries you might sustain arising out of this program and you will be required to indemnify, hold harmless, and defend Buffalo Rehab Group for any claims arising out of participation in said program(s).
Risk of Injury - "As a participant in the program, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of injuries, including death, damages, or loss which I may sustain as a result of participating in any and all activities associated with this program".
Waiver of Injury Claims -"l agree to waive and relinquish any and all claims I may have arising out of, connected with, or in any way associated with the activities of the program."
Release from Liability - "I do hereby fully release and discharge Buffalo Rehab Group and its officers, agents, and employees from any and all claims from injuries, including death, damage or loss which I may have or which may occur on account of participation in the program."
Indemnity and Defense - "I further agree to indemnify, hold harmless and defend Buffalo Rehab Group and its officers, agents, and employees from any and all claims from injuries, including death, damages and losses sustained by me and arising out of, connected with, or in any way associated with the activities of the program."
I have read and fully understand and agree to the above Participants Liability Waiver and Hold Harmless Agreement.

MANDATORY LIABILITY WAIVER TO PARTICIPATE
WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT: Villa Maria Flag Football Camp
To be signed by parent or legal guardian of the Villa Maria Flag Football Camp participant.
1. In consideration for receiving permission to participate in the activities of the Villa Maria Flag Football Camp, I hereby release, waive, discharge and indemnify the Villa Maria College and its officers, trustee, employees, agents, successors and assigns, (“Releasees”) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in any of the Villa Maria Flag Football Camp activities, whether caused by the negligence of the Releasees or otherwise.
2. I am fully aware of the risks involved and hazards connected with the Villa Maria Flag Football Camp’s activities, including the risk of injury or death. I elect to voluntarily allow my minor child to participate in these activities with full knowledge that they may be hazardous. I voluntarily assume the risk of loss, property damage, personal injury, and/or death that may be sustained by my child by engaging in such activities, whether caused by the negligence of Releasees or otherwise.
3. I further herby agree to indemnify, defend and hold harmless the Releasees from any loss, liability, damage or costs, including court costs and attorney fees, that they may incur due to my child’s participation in the Villa Maria Flag Football Camp’s activities, whether caused by the contributory negligence of Releasees or otherwise.
4. I understand that the Villa Maria Flag Football Camp does not maintain insurance policy covering liability arising from my child’s participation in the Villa Maria Flag Football Camp’s activities. I am aware that I should review my own personal insurance coverage as no such losses or liability will be covered by any insurance maintained by the team.
5. I understand that this Waiver of Liability and Hold Harmless Agreement binds my me, my child, and the members of my family and spouse as well as my heirs, or personal representative if I am deceased and shall be deemed as a release, waiver, discharge and covenant not to sue the above-named Releasees and as an agreement to indemnify, defend and hold the Villa Maria Flag Football Camp harmless against any claims. This Agreement shall be construed in accordance with the laws of the State of New York.
6. In signing this release, I acknowledge and represent that I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed; no oral representative, statements, or inducements, apart from the foregoing written agreement, have been made; I am at least eighteen (18) years of age and fully competent; and I execute this release for full, adequate and complete consideration fully intending to be bound by it.
7. This Waiver of Liability and Hold Harmless Agreement shall remain in effect into perpetuity.
8. It is necessary to submit this form only one time.
